There are currently two glaucoma schemes running in Dorset. The first is a direct referral scheme for suspected new cases of glaucoma. The second is a shared eye care scheme for patients with stable glaucoma, suspect glaucoma or ocular hypertension.
In this enhanced service, accredited ophthalmic performers can refer patients directly to the Hospital Eye Service after performing additional tests to refine the referral. Patients can be referred to the following Hospital Eye Services:
Bournemouth Eye Unit
Royal Bournemouth Hospital
Castle Lane East
Poole Hospital NHS Foundation Trust
This scheme is designed for patients with ocular hypertension, suspect glaucoma or stable well controlled glaucoma.
The hospital sends a green form to the patient concerned, together with a list of all the practitioners on the scheme. The green form lists the tests that the optometrist will need to perform if appropriate - IOP's, fields, C/D ratios, anterior chamber assessment etc. It is suggested that the patient takes the form to the practice of their choice so that a reminder can be sent to make an appointment at the required time. Once completed, the form is sent to the Bournemouth Eye Unit for the consultant to decide on appropriate ongoing management. The patient is informed by the Eye Unit when the next follow up is due, or they are recalled to the hospital for further assessment.
